Do I need a doctor’s referral to make an appointment?

No, you do not need a referral to receive any services from Sage Physical Therapy or Sage Herbal Consulting. You may contact Dr. Carestio directly for all services.

What is cash-based health care?

When you receive care from a provider who operates a cash-based practice, you are freed from the constraints of the bureaucratic insurance system. You and your health care provider create a treatment plan that will best serve your health care needs.

You can still submit claims to your health insurance provider, but your care is no longer driven by the insurance model of care. You will be expected to pay for services at the time they are received.

Will you provide paperwork that I can submit to my insurance provider?

Yes, you will be provided a detailed receipt that you can submit to your private insurance provider. However, there is no guarantee that your insurance company will reimburse you for any payments.

Please note that Sage Physical Therapy has no relationship with Medicare and due to Medicare regulations, you may not receive Medicare approved services from Sage Physical Therapy.

Do I have to be a patient at Premier Birth Center to see you?

No, anyone may make an appointment with Dr. Carestio for physical therapy, wellness, yoga and herbal consultation services.

Do you only see women?

Sage Physical Therapy focuses on women’s health, but men are welcome to make an appointment for physical therapy, wellness, yoga and herbal consultation services. See LGBTQ link for more information about care within the Queer community.
